OBJECTIVES: Previous literature has primarily viewed physical effort as an aversive experience. However, recent research suggests that effort can also be valued positively. These differences in approach and avoidance tendencies toward physical effort may play a key role in the self-regulation of physical activity behaviors. The aim of this study was to develop a scale that measures these tendencies and contributes to a better understanding of physical effort and how it affects behavior. METHODS: The Physical Effort Scale (PES) was developed in Study 1 based on expert evaluations (n = 9) and cognitive interviews (n = 10). In Study 2 (n = 680, 69% female), content validity and dimensional structure were examined using principal component analysis and confirmatory factor analysis. Item reduction was conducted using item response theory. Preliminary construct validity was explored using regression. Study 3 (n = 297, 71% female) was used to validate dimensional structure, internal consistency, and construct validity, and to assess test-retest reliability. RESULTS: In Study 1, 44 items were rated for content validity, of which 18 were selected and refined based on cognitive interviews. Analyses from Study 2 allowed reducing the scale to 8 items with a two-dimension structure: tendency to approach (n = 4) and to avoid physical effort (n = 4). The two subscales showed high internal consistency (α = 0.897 for the approach dimension and 0.913 for the avoidance dimension) and explained usual levels of physical activity, providing preliminary evidence of construct validity. Study 3 confirmed the two-dimension structure with high internal consistency (α = 0.907 and 0.916 for the approach and avoidance dimension, respectively) and revealed acceptable test-retest reliability (intraclass correlation >0.66). Patterns of associations with other constructs showed expected relationships, confirming the concurrent, convergent, and discriminant validity of the scale. CONCLUSIONS: The PES is a valid and reliable measure of individual differences in the valuation of physical effort. This scale can assess the propensity to engage in physically demanding tasks in non-clinical populations. The PES and its manual are available in the Supplementary Material.

RESUMO

(1) Infrared thermography of the inner canthus of the eye has emerged as a promising tool for temperature screening and fever diagnosis. Its non-invasive nature lends itself well to mass screening in diverse settings such as schools, public transport, and healthcare facilities. Swift and accurate temperature assessment plays a pivotal role in the early identification of potential fever cases, facilitating timely isolation, testing, and treatment, thereby mitigating the risk of disease transmission. Nonetheless, the reliability of this approach in the pediatric population, especially when compared to conventional thermometry methods, remains unexplored. This preliminary study aimed to evaluate the concordance between the temperature of the inner canthus of the eye (Tic,eye), referred to as the brain-eyelid thermal tunnel (BTT°), with axillary and tympanic methods in afebrile children. (2) Methods: A cohort of 36 children, matched in a 1:1 ratio for gender and age, underwent comprehensive assessments encompassing anthropometric data, blood pressure evaluations, axillary (Tax) and tympanic (Tty) temperature measurements, as well as BTT° infrared thermography. (3) Results: The findings revealed a high level of concordance among the tympanic, axillary, and BTT° measurement methods. Bland-Altman plots showed that the bias was minimal, and no statistically significant differences were observed when comparing BTT° with axillary (p = 0.136) and tympanic (p = 0.268) measurements. Passing-Bablok regression scatter plots further confirmed the agreement, aligning the fitted regression line closely with the identity line for both axillary versus BTT° and tympanic (Tty) versus BTT° comparisons. (4) Conclusions: This study holds significant implications for public health, especially in the context of infectious disease outbreaks such as COVID-19. BTT° infrared thermography of the inner canthus of the eye (Tic,eye) reliably measures body temperature in afebrile children in controlled settings; nevertheless, its practical application necessitates the adaptation of biothermodynamic parameters to accommodate diverse environmental conditions.

Introducción: La implicación de los médicos asistenciales en tareas de investigación clínica presenta grandes diferencias entre las diversas instituciones. Material y método: Estudio transversal basado en una encuesta enviada a los miembros del Grupo de Trabajo de Enfermedades Infecciosas (GTei) de la Sociedad Española de Medicina Interna (SEMI) durante el mes de mayo de 2022. Resultados: De 1.789 miembros del GTei, 169 miembros (9,45%) cumplimentaron la encuesta. La percepción de la conveniencia de participación de cada facultativo en una o varias líneas de investigación fue de 8 puntos (P25:7; P75: 9 puntos). La percepción sobre el estímulo para investigar del sistema sanitario o de la dirección del hospital fue de 2 puntos (1-4), respectivamente. El apoyo a la investigación fue valorado con de 5 (2-7) y 6 (3-7) puntos en relación con el jefe de servicio y los compañeros del departamento, respectivamente. Otros factores evaluados fueron la falta de tiempo por no poder reducir la actividad asistencial (9; 7-10 puntos), la priorización de las actividades de ocio durante el tiempo libre disponible (7; 5-8 puntos), la organización de la carga asistencial (6; 3-9 puntos), las dificultades en la coordinación con otros servicios clínicos o centrales (6; 5-7 puntos y 6; 5-8 puntos, respectivamente). Conclusiones: La investigación clínica es muy bien valorada por los internistas dedicados a la patología infecciosa. Las principales necesidades percibidas son un mayor apoyo institucional y de la dirección del hospital, una mejor organización del departamento, la coordinación interdepartamental y disponer de más tiempo para esta actividad.(AU)


Background: The involvement of attending physicians in clinical research activities differs greatly among institutions. Method: Cross-sectional study based on a survey submitted to the members of the Working Group on Infectious Diseases (GTei) of the Spanish Society of Internal Medicine (SEMI) during the month of May 2022. Results: Out of 1,789 members of the GTei, 169 members (9.45%) completed the survey. The perception of the convenience of participation of each physician in one or more lines of research was 8 points (P25:7; P75: 9 points). The perception of encouragement to do research by the health system or hospital management was 2points (1-4), respectively. Support for research was rated at 5 (2-7) and 6 (3-7) points inrelation to the head of service and colleagues in the department, respectively. Other factors evaluated were the lack of time due to not being able to reduce the care activity (9; 7-10 points), prioritization of leisure activities during available free time (7; 5-8 points), organization of the care load (6; 3-9 points), difficulties in coordinating with other clinical or central services (6; 5-7 points and 6; 5-8 points, respectively). Conclusions: Clinical research is highly valued by internists dedicated to infectious diseases. The main perceived needs are greater institutional and hospital managementsupport, better organization of the department, interdepartmental coordination and more time for this activity. (AU)

The serum level of trimethylamine N-oxide (TMAO), a gut microbiota metabolite associated with diabetes, cancer, inflammatory and neurological diseases, can be determined by the micro-enzyme-linked immunosorbent assay (ELISA) method. However, we had problems obtaining accurate standard curves with the original kit protocol from Bioassay Technology Laboratory. We aimed to acquire proper standard curves by modifying the kit protocol in this study. First, we evaluated the human TMAO ELISA kit protocols and other human ELISA kits. We maintained the incubation times longer and increased the wash cycle. Moreover, we incubated the standards containing biotinylated antibody in the wells alone. Then we washed the wells and added streptavidin-HRP for the second incubation step. The data of original and modified ELISA kit protocol were analyzed with Student's t-test. We measured higher absorbance with lower standard solution concentration in experiments that followed the original kit protocol. After investigating other human TMAO ELISA kits, we noticed that the SunRed Biotechnology Company and MyBioSource companies suggested similar protocols to the Bioassay Technology Laboratory company. The ELK Biotechnology ELISA protocol was different from others. However, since there is no biotinylated antibody in the standard solution in the ELK biotechnology kit, we changed some steps by examining other human ELISA protocols from different companies. After performing the modified protocol, we found that the absorbances of the standard solutions were consistent with their concentrations, and we obtained an accurate standard curve. Higher R2 values and lower absolute difference of standard concentrations were found in the modified kit protocol. The human TMAO ELISA protocol, which we modified in this study, will enable researchers to obtain more reliable results and prevent them from failing time and resources.

Objectives: Pseudomyxoma peritonei (PMP) is a rare cancer currently affecting over 11,736 patients across Europe. Since PMP is so uncommon, collaboration between scientific centers is key to discovering the mechanisms behind the disease, efficient treatments, and targets pointing to a cure. To date, no consensus has been reached on the minimum data that should be collected during PMP research studies. This issue has become more important as biobanking becomes the norm. This paper begins the discussion around a minimum data set that should be collected by researchers through a review of available clinical trial reports in order to facilitate collaborative efforts within the PMP research community. Content: A review of articles from PubMed, CenterWatch, ClinicalTrials.gov and MedRxiv was undertaken, and clinical trials reporting PMP results selected. Summary: There is a core set of data that researchers report, including age and sex, overall survival, peritoneal cancer index (PCI) score, and completeness of cytoreduction, but after this, reports become variable. Outlook: Since PMP is a rare disease, it is important that reports include as large of a number of standardised data points as possible. Our research indicates that there is still much ground to cover before this becomes a reality.

Introducción. La innovación ha marcado el progreso de la cirugía por medio del desarrollo y aplicación de procedimientos seguros y rentables enfocados en solucionar los problemas del profesional. Además, ofrece a los usuarios nuevas técnicas y alternativas accesibles y seguras para elegir. División de temas tratados. La innovación en cirugía se basa en: 1) técnicas; 2) mejora en la logística de la práctica; 3) e introducción, diseño de dispositivos y herramientas. El Innovar debe estructurarse basado en marcos propuestos como el Idea, Development, Exploration, Assessment, Long term study (IDEAL). Hay que comprender la necesidad de innovar en cirugía y proponer puntos clave para tener en cuenta durante la implementación del marco IDEAL, herramienta valiosa como propuesta innovadora de cirugía en Latinoamérica. Conclusiones. La innovación es un camino al progreso y avance de la medicina en general. Se propone que para lograr un impacto innovador se centre en la aplicación del algoritmo propuesto, relacionándolo con la realidad que afronta nuestra sociedad Latinoamericana.


Introduction. Innovation has driven progress in surgery through the development and application of safe and profitable procedures that focus on solving the professional's problems. It also offers users new choices of safe and accessible techniques and alternatives. Division of Covered Topics. Innovation in surgery is based on 1) techniques; 2) improved logistics in the practice, and 3) the design and introduction of devices and tools. Innovation should be structured in frameworks such as those proposed by IDEAL (Idea, Development, Exploration, Assessment, Long term study). It is necessary to understand the need to innovate in surgery and to propose key aspects to take into consideration during the implementation of the IDEAL framework, which represents a valuable tool for the development of innovative proposals in Latin America. Conclusions. Innovation is a path towards progress and the advancement of medicine in general. It is suggested that innovative impact can be achieved by using the proposed algorithm, associating it with the realities we face in Latin America.


Introdução. A inovação tem marcado o avanço da cirurgia através do desenvolvimento e aplicação de procedimentos seguros e rentáveis com foco na solução dos problemas do profissional. Além disso, ofrece aos usuários novas técnicas e alternativas acessíveis e seguras para escolher. Divisão dos tópicos abordados. A inovação em cirurgia baseia-se em: 1) técnicas; 2) melhoria na logística da prática; e 3) introdução, design de dispositivos e ferramentas. Inovar deve ser estruturado com base em estruturas propostas, tais como Idea, Development, Exploration, Assessment, Long term study (IDEAL). É preciso entender a necessidade de inovar na cirurgia e propor pontos-chave a serem considerados na implantação da estrutura IDEAL, uma ferramenta valiosa como proposta inovadora da cirurgia na América Latina. Conclusões. A inovação é um caminho para o progresso e avanço da medicina em geral. Propõe-se que, para alcançar um impacto inovador, se concentre na aplicação do algoritmo proposto, relacionando-o com a realidade que enfrenta nossa sociedade Latino-americana.

Purpose: The aim of this prospective cohort study was to assess the facial-palatal ridge dimensional changes that occurred at 8-10 months following flapless dual-zone (DZ) immediate implant placement and socket-shield (SS) immediate placement in the maxilla. Materials and Methods: A total of 19 patients who received 20 implants were included, with 10 implants (MegaGen AnyRidge) placed for each treatment, DZ and SS. Cast models were made at 8-10 months after implant placement to assess the dimensional ridge changes by measuring the facial-palatal ridge width on implant sites (T) at six designated points starting from the gingival margin (0, 1, 2, 3, 5 and 7) and comparing it with the corresponding measurement on the contralateral tooth site (C). Results: All 20 implants demonstrated successful osseointegration at 8-10 months follow-up (survival rate 100%). Two out of ten cases of SS group presented with minor manageable complications of external shield exposure. DZ group showed an average of 0.3 mm facial-palatal ridge width reduction, while SS group revealed a mean gain in ridge width of approximately 0.2 mm at 8-10 months follow-up. Conclusion: When comparing between two treatments, there was a significant difference in ridge width change at 8-10 months following implant placement, indicating better preservation of the ridge contour for socket shield technique.

Objetivo Caracterizar los modos de enseñar e investigar políticas públicas a nivel de posgrado en Colombia. Métodos Investigación cualitativa de tipo análisis documental. Inicialmente, se realiza una caracterización general de autores, enfoques teóricos empleados, temáticas de interés, entre otros. En un segundo momento, y a partir del contenido de los programas académicos de los posgrados de donde emergieron las tesis analizadas, se identificaron significantes centrales del discurso curricular. Resultados Se analizaron 313 tesis y 8 programas de posgrado en políticas públicas. Se destaca una marcada preferencia por el empleo de enfoques pospositivistas y estrategias tipo problemsolving; cuestionamientos débiles a determinantes estructurales de opresión, exclusión y explotación, y mantenimiento de relaciones de dominación en la toma de decisiones entre investigadores de ciencias políticas/administrativas y sujetos con otros saberes. Conclusiones Los hallazgos del estudio problematizan el campo del Análisis de Políticas Públicas, sus posturas epistemológicas y métodos predominantes, así como el papel que asume el analista, invitando al lector a reflexionar sobre la necesidad de incursionar en enfoques críticos alternativos.


Objective To describe the methods used at a postgraduate level to teach and research public policies in Colombia. Methods Documentary analysis as part of a qualitative study. Among other things, a broad characterisation of authors, theoretical techniques employed, and areas of interest was accomplished in a first instance. Then, the main significants of the programmatic discourse were recognized based on the postgraduate curriculum from the universities from which the investigated thesis came. Results 313 theses and 8 postgraduate programs in public policy analysis were studied. There is a clear preference for post-positivist mainstream approaches and problem-solving strategies; weak challenges to structural determinants of oppression, exclusion, and exploitation; and the preservation of dominant relationships in decision-making between political/administrative science researchers and subjects with other knowledge. Conclusions The research's findings challenge the discipline of Public Policy Analysis, its epistemological views, and its dominant study methodologies, as well as the analyst's role, encouraging the reader to consider the need for alternative critical approaches.

BACKGROUND: Early developmental impairment (EDI) is common and has many aetiologies and, therefore, potential investigations. There are several published guidelines recommending aetiological investigations, and paediatricians' views of them varies. Little is known on the thought processes underlying clinical decisions in investigating EDI. This study aimed to describe the thought processes affecting clinical decisions on the investigation of EDI within a nationalised health care system. METHODS: A qualitative descriptive study using semi-structured qualitative interviews performed in person or via video link with paediatricians who see children with EDI in England. As part of the interview, a case study of a fictional disease, Cavorite deficiency, modelled on biotinidase deficiency, was given to participants with the cost of testing, incidence and likelihood it would respond to treatment. This allowed exploration of cost without encumbrance from predisposing views and training on the condition. Thematic analysis was performed by iterative approach. Where participants stated they wanted to redirect money from investigations to treatment, were that even possible, we asked which services they would like to be better funded in their area. RESULTS: Interviews were conducted with 14 consultant paediatricians: 9 Community / Neurodisability, 2 General paediatricians, and 3 Paediatric Neurologists. Two themes were identified: the value of an aetiological diagnosis to families and managing risk and probability when investigating EDI. The latter contained 4 subthemes: 'circumspection' involved blanket investigations chosen irrespective of phenotype and high regard for guidelines; 'accepting appropriate risk' involved participants choosing investigations based on clinical phenotype, recognising some aetiologies would be missed; consultants found they 'transitioned between practices' during their career; and 'improved practice' was thought possible with better evidence on how to stratify investigations based on phenotype. Services that were most frequently reported to need additional funding were therapy services, early community developmental services, management of behaviour, sleep and mental health, and educational support. CONCLUSIONS: There are many factors that influence paediatricians' choice of aetiological investigation in EDI, but clinical factors are the most important. Paediatricians want better evidence to allow them to select the right investigations for each child without a significant risk of missing an important diagnosis.

BACKGROUND: HIV disproportionately impacts Black men who have sex with men (MSM), and targeting the primary relationship (ie, couples) using mobile technology for health holds promise for HIV prevention. Web-based recruitment of MSM is commonly employed in HIV prevention and intervention research. However, little known about recruiting Black MSM couples on the internet in the United States. OBJECTIVE: This study describes the process of recruiting Black MSM couples over social networking and dating apps frequented by MSM. We describe the activities for recruiting, screening, and enrolling participants as part of a randomized trial employing a multipronged recruitment approach. METHODS: Black MSM in couples were recruited via three apps (ie, Jack'd, Adam4Adam, and Growlr) between May 2020 and March 2021 during the COVID-19 pandemic in the United States. Black MSM couples were eligible if one or both partners are Black, MSM, and living with HIV, and if both partners were 18 years or older, and have been together for at least 2 months in what they both consider a primary relationship (ie, one in which both partners reported feeling most committed to over any other partner or relationship). RESULTS: A total of 10 Black MSM couples (n=20) were enrolled via social networking apps. App recruitment activities were a combination of passive (eg, in-app advertisements) and active (eg, direct messaging of users) engagement. Recruitment approaches varied by the social networking app owing to differences in app features. A full-time recruiter experienced challenges such as bugs (ie, technical errors in computer program or system), navigating technical requirements specific to each app, and web-based harassment. CONCLUSIONS: Despite challenges, it was possible to recruit Black MSM couples virtually into research as part of a multipronged recruitment strategy. We identify tips for using web-based dating and other social networking apps as part of a recruitment strategy in future research with Black MSM couples.

Artificial intelligence in diagnostic cardiac-imaging platforms is advancing rapidly. In particular, artificial intelligence algorithms have increased the efficiency and accuracy of echocardiographic cardiovascular imaging, resulting in more complex echocardiographic imaging techniques and expanded use among noncardiologists. Here, we provide an overview of real-world applications of artificial intelligence in echocardiography including automatic high-quality computer-optimized image acquisition sequences, automated measurements, and algorithms for the rapid and accurate interpretation of cardiac physiology. These advances will not replace physicians but will improve their productivity, workflow, and diagnostic performance.

Artificial intelligence and machine learning are rapidly gaining popularity in every aspect of cardiovascular medicine. This review discusses the past, present, and future of artificial intelligence in education, remote proctoring, credentialing, research, and publication as they pertain to cardiovascular procedures. This review describes the benefits and limitations of artificial intelligence and machine learning and the exciting potential of integrating advanced simulation, holography, virtual reality, and extended reality into disease diagnosis and patient care, as well as their roles in cardiovascular research and education. Nonetheless, much of the available data resides in electronic medical records or within industry-sponsored proprietary programs that are not compatible or standardized for current clinical application. Many areas in cardiovascular medicine would benefit from the introduction or increased use of artificial intelligence. Web-based artificial intelligence applications could be used to address unmet needs for education, on-demand procedural proctoring, credentialing, and recredentialing for interventionists and physicians in remote locations. Further progress in artificial intelligence will require further collaboration among computer scientists and researchers in order to identify and correct the most relevant problems and to implement the best data-based approach to achieving this goal. The future success of artificial intelligence in cardiovascular medicine will depend on the degree of collaboration between all pertinent experts in this field. This will undoubtedly be a prolonged, stepwise process.

We intended to precisely evaluate the dietary quality of male and female medical college students using canteen data from the "Intelligent Ordering System" (IOS), combined with the supplemental food frequency questionnaire (SFFQ) and the Chinese Healthy Eating Index (CHEI) in Shanghai, China, to explore the potential factors influencing dietary quality. A total of 283 college students with the average age of 24.67 ± 3.21 years and Body Mass Index of 21.46 ± 3.49 kg/m2 in the medical school were enrolled in this study, and an online questionnaire investigation was conducted to collect the general information, lifestyle behavior, and SFFQ. The dietary data of the study participants from the school canteen were exported from the IOS of the Information Office of Fudan University. The CHEI consists of 17 components and the total score is 100, with a maximum score of each component of 5 or 10. We calculated each component score of the CHEI and aggregated the total score for male and female study participants. The Chi-square test and Wilcoxon rank sum test were employed in comparing the differences between the demographic characteristics and CHEI component scores of males and females. Univariate and multiple linear regression models were employed to examine the potential influencing factors of the total CHEI score. The CHEI median score was 66.65, and the component score for total grains was relatively low. Added sugars was the most overconsumed CHEI component. There were relatively serious deficiencies, based on the CHEI component scores, in fruits, soybeans, fish and seafood, and seeds and nuts in both sexes. Females had significantly higher CHEI scores than males (68.38 versus 64.31). The scores for tubers, total vegetables, dark vegetables, fruits, fish and seafood, dairy, and red meats were significantly higher in females than in males. Influencing factors including sex, education, dietary health literacy, and amount of time spent sedentarily were significantly associated with CHEI score. Our research revealed that the overall dietary quality needs to be further improved for college students at the medical school in Shanghai, China, with low intakes of total grains, fruits, soybeans, fish and seafood, and seeds and nuts, and high intakes of added sugars. Compared with males, females showed higher diet quality and conformed more strongly with the recommended Dietary Guideline for Chinese. Education, dietary health literacy, and amount of time spent sedentarily should be noted for improving the dietary quality of college students.

PURPOSE: To assess parent perspectives of the current and potential future tests for their child with inflammatory bowel disease (IBD). METHODS: New Zealand parents of a child with IBD were invited to complete an anonymous online survey. Experiences relating to their child's blood or faecal tests, medical imaging (abdominal ultrasound [US], abdominal computerised tomography [CT] and magnetic resonance enterography) and colonoscopy were collected. Perceived attitudes to potential future testing of urine, saliva, and breath, were sought. RESULTS: Twenty-eight parents, 93% female completed the survey, and 86% were aged between 35 and 54 years. Baseline information was provided by parents for 27 of 28 children, 70.3% had Crohn's disease with a mean disease duration of 2.67 years. Blood tests were the most requested and completed tests, while CT was the least ordered and most refused test. Colonoscopy was rated as the least comfortable and generated the most worry. Explanation of test significantly improved parent's levels of understanding when their child had blood, faecal, imaging (US) or colonoscopy tests. Providing an explanation, test invasiveness and the impact of the blood results may have on their child's treatment significantly improved parents' comfort levels. However, explanation of colonoscopy generated a significant parental concerns. Saliva, urine and blood tests were chosen as the most preferred disease monitoring tests. CONCLUSION: Parents preferred any tests less invasive than colonoscopy for monitoring their child's IBD. Although providing explanation of their child's tests enhanced parents' understanding, it can also affect parents' levels of concern and comfort.

Background: Human immunodeficiency virus (HIV)-related mortality remains high in sub-Saharan Africa. Clinical autopsies can provide invaluable information to help ascertain the cause of death. We aimed to determine the rate and reasons for autopsy refusal amongst families of HIV-positive decedents in Uganda. Methods: We consented the next-of-kin for post-mortem examinations among Ugandan decedents with HIV from 2017-2020 at Kiruddu National Referral Hospital. For those who refused autopsies, reasons were recorded. Results: In this analysis, 165 decedents with HIV were included from three selected wards at Kiruddu National Referral Hospital.  Autopsy was not performed in 45% of the deceased patients; the rate of autopsy refusal was 36%. The most common reasons for autopsy refusal were time constraints (30%), family satisfaction with clinical diagnosis (15%), fear of disfigurement of the remains (15%), and lack of perceived benefit (15%). By seeking consent from multiple family members and clearly explaining to them the purpose of performing the autopsy, we found a reduction in the rate of autopsy refusal among relatives of the deceased patients at this hospital compared to previous studies at the same site (36% vs. 60%). Conclusions: We found lower rates of autopsy refusal compared to previous studies at the same site. This underscores the importance of clearly explaining the purpose of autopsies as they increase active sensitization about their relevance and dispel myths related to autopsies among the general population. Good, culturally sensitive, and timely explanations to the family of the benefits of autopsy increase the rate of obtaining permission. Building capacity for performing autopsies by training more pathologists and increasing laboratory resources to decrease the turn-around-time for autopsy reports and extending these services to peripheral health facilities could improve autopsy acceptance rates.

BACKGROUND: It is recognised that medical tests are overused in primary care; however, it is unclear how best to reduce their use. AIM: To identify which strategies are effective in reducing the use of low-value medical tests in primary care settings. DESIGN AND SETTING: Systematic review. METHOD: The databases MEDLINE, EMBASE, and Rx for Change were searched (January 1990 to November 2019) for randomised controlled trials (RCTs) that evaluated strategies to reduce the use of low-value medical tests in primary care settings. Two reviewers selected eligible RCTs, extracted data, and assessed their risk of bias. RESULTS: Of the 16 RCTs included in the review, 11 reported a statistically significant reduction in the use of low-value medical tests. The median of the differences between the relative reductions in the intervention and control arms was 17% (interquartile range 12% to 24%). Strategies using reminders or audit/feedback showed larger reduction than those without these components (22% versus 14%, and 22% versus 13%, respectively) and patient-targeted strategies showed larger reductions than those not targeted at patients (51% versus 17%). Very few studies investigated the sustainability of the effect, adverse events, cost-effectiveness, or patient-reported outcomes related to reducing the use of low-value tests. CONCLUSION: This review indicates that it is possible to reduce the use of low-value medical tests in primary care, especially by using multiple components including reminders, audit/feedback, and patient-targeted interventions. To implement these strategies widely in primary care settings, more research is needed not only to investigate their effectiveness, but also to examine adverse events, cost-effectiveness, and patient-reported outcomes.

INTRODUCTION: In epithelial ovarian cancer (EOC), consensus regarding optimal use of PET/CT is lacking. Limited evidence suggests its accuracy in preoperative staging, investigating recurrence and predicting optimal secondary debulking. This study evaluated indications for PET/CT, impact of PET/CT results on EOC management, and its added value over conventional imaging. METHODS: Patients with EOC aged 19 years and older who underwent PET/CT at BC Cancer Vancouver between January 2007 and September 2017 were eligible. Medical records were retrospectively reviewed with the following data abstracted: patient demographics, PET/CT indications and results, recent conventional imaging results, and application of PET/CT findings. Basic descriptive analysis was performed to determine most common use of PET/CT and its management impact. RESULTS: 257 PET/CTs in 216 patients were eligible. Most PET/CTs (135/257, 52.6%) were performed for suspected disease recurrence/progression. Management after PET/CT changed in 1/3 of cases with the greatest impact seen when assessing suitability in recurrent disease for focal radiotherapy or secondary surgical debulking. For 106/257 cases with recent conventional imaging available, although differences in disease extent were found in ~50%, clinical conclusions drawn were the same in ~75%. DISCUSSION: Although PET/CT is most commonly used to investigate suspected recurrence/progression, its impact on management here is low. Greater impact is seen when considering localized therapy in EOC recurrence. When compared with recent conventional imaging, although PET/CT may detect differences in disease extent, clinical conclusions are frequently the same. CONCLUSION: Impact of PET/CT on management is greatest in EOC recurrence when considering local therapies, beyond which clinical presentation and conventional imaging may suffice.

Para contribuir com a atenção primária à saúde, a pesquisa em medicina de família e comunidade precisa evitar quatro fatores que têm levado ao desperdício da pesquisa biomédica em nível mundial: questões de pesquisa irrelevantes; métodos inadequados para alcançar os objetivos da pesquisa; lentidão e inadequação da publicação dos resultados; relato da pesquisa obscuro e pouco transparente. Neste editorial, introduzimos medidas para os autores garantirem o impacto de sua pesquisa, e apresentamos novas políticas editoriais da RBMFC.


To contribute to primary health care, research in family and community medicine needs to avoid four factors that have led to wasted biomedical research worldwide: irrelevant research questions; inadequate methods to achieve the research objectives; slow and inadequate publication of results; obscure and not transparent reporting. In this editorial, we introduce measures for authors to ensure the impact of their research, and new editorial policies from RBMFC.


Para contribuir a la atención primaria de salud, la investigación en medicina familiar y comunitaria debe evitar cuatro factores que han llevado a la pérdida de investigación biomédica en todo el mundo: preguntas de investigación irrelevantes; métodos inadecuados para lograr los objetivos de investigación; lentitud y publicación inadecuada de resultados; oscuridad e falta de transparencia del informe de investigación. En este editorial, presentamos medidas para que los autores garanticen el impacto de su investigación, y presentamos nuevas políticas editoriales de RBMFC.
